  1. You must become a California Community College student for these units to be recorded. Complete WLAC’s online application at: http://www.laccd.edu/Students/opencccapply/applywlac/Pages/default.aspx.
  1. After you receive your WLAC student ID number, complete the top portion of this form. Complete the following fields on the form:

    • Name
    • WLAC Student ID
    • Email
    • Major (i.e. Certificate of Achievement in EMT, Certificate of Achievement in Paramedicine, or Associate of Science Degree in Paramedicine)
  1. Email the partially completed form with the information above to:

Aaron Benitez, RDHAP, MSDH
Articulation Officer, West Los Angeles College
WLAC-Articulation@laccd.edu

  1. After discussing your academic credit request with the Articulation Officer, make an appointment with a WLAC counselor who will create a WLAC Student Education Plan: http://www.wlac.edu/counseling/index.aspx.
  1. After meeting with the WLAC counselor, request an official transcript be sent from the UCLA Center for Prehospital Care to WLAC: https://www.cpc.mednet.ucla.edu/services/transcripts. Do not request the transcript before meeting with the counselor at WLAC.
  1. After reviewing the transcript and submitted paperwork, WLAC will record the units on the student’s California Community College transcript.
